Description

Indications
- Treatment of metastatic non-small cell lung cancer (NSCLC) with ALK-positive tumors
- Treatment of metastatic NSCLC with ROS1-positive tumors
Pharmacology
- Inhibitor of receptor tyrosine kinases including ALK, c-Met, ROS1, and RON
- Demonstrated concentration-dependent inhibition of ALK, ROS1, and c-Met phosphorylation
- Absorption: Peak concentration achieved in 4-6 hours
- Distribution: Extensive tissue distribution with high plasma protein binding
- Metabolism: Predominantly metabolized by CYP3A4/5
- Elimination: Mean half-life of 42 hours
Dosage administration
- Recommended Dose: 250 mg orally, twice daily
- Geriatric Use: No safety or efficacy differences observed in older patients
- Pediatric Dose: Safety and effectiveness not established for pediatric patients
- Renal Impairment: 250 mg orally, once daily for severe renal impairment
- Hepatic Impairment: Caution advised
Drug interaction
- CYP3A Inhibitors: Concurrent use should be avoided
- CYP3A Inducers: Concurrent use should be avoided
- CYP3A Substrates: Concurrent use should be avoided
Side effects
- Common adverse reactions include vision disorders, nausea, and diarrhea
Pregnancy lactation
- Can cause fetal harm
- Females of reproductive potential should use effective contraception
- Patients should not breastfeed during treatment and for 45 days after the final dose
Precautions warnings
- Periodic liver testing recommended
- Permanently discontinue in patients with ILD/Pneumonitis
- Monitor ECG and electrolytes in patients with QTc prolongation
