Description

Indications
- Treatment of rheumatoid arthritis in adults
- Treatment of psoriatic arthritis in adults
- Treatment of ulcerative colitis in adults
Pharmacology
- Inhibition of janus kinase (JAK) signaling pathway to decrease inflammatory response
Dosage & Administration
- Recommended dosage for rheumatoid arthritis: Tofacitinib 5 mg twice daily or Tofacitinib 11 mg once daily
- Recommended dosage for psoriatic arthritis: Tofacitinib 5 mg twice daily or Tofacitinib 11 mg once daily
- Recommended dosage for ulcerative colitis: Tofacitinib 10 mg twice daily for at least 8 weeks, then 5 or 10 mg twice daily
Interactions
- Interaction with potent inhibitors of Cytochrome P450 3A4 (CYP3A4)
- Interaction with potent CYP inducers
Side Effects
- Commonly reported adverse reactions include upper respiratory tract infections, headache, diarrhea, and nasopharyngitis
Pregnancy & Lactation
- Pregnancy Category C
- Use during pregnancy only if potential benefit justifies potential risk to the fetus
- Unknown excretion in human milk
Precautions & Warnings
- Avoid use during active serious infection
- Use with caution in patients at increased risk of gastrointestinal perforations
- Recommended laboratory monitoring for potential changes in lymphocytes, neutrophils, hemoglobin, liver enzymes, and lipids
Use in Special Populations
- Safety and effectiveness in pediatric patients not established
- Caution in treating the elderly due to higher incidence of infections
- Half the total daily dosage recommended for patients with impaired renal and hepatic function
