Indications

  • Rheumatoid Arthritis treatment for adults with inadequate response to methotrexate
  • Psoriatic Arthritis treatment for adults with inadequate response to methotrexate
  • Ulcerative Colitis treatment for adults with moderate to severe active condition

Pharmacology

  • Autoimmune disease characterized by dysregulation of pro-inflammatory cytokines
  • Tofacitinib is a partial and reversible janus kinase (JAK) inihibitor
  • Prevents body from responding to cytokine signals
  • Inhibits JAK-STAT pathway to decrease inflammatory response

Dosage & Administration

  • Tofacitinib 5 mg twice daily for Rheumatoid Arthritis
  • Tofacitinib 11 mg once daily for Rheumatoid Arthritis
  • Tofacitinib 5 mg twice daily for Psoriatic Arthritis
  • Tofacitinib 11 mg once daily for Psoriatic Arthritis
  • Tofacitinib 10 mg twice daily for Ulcerative Colitis

Interaction

  • Interaction with potent inhibitors of Cytochrome P450 3A4 (CYP3A4)
  • Interaction with moderate inhibition of CYP3A4 and potent inhibition of CYP2C19
  • Interaction with potent CYP inducers

Side Effects

  • Commonly reported adverse reactions: upper respiratory tract infections, headache, diarrhea, nasopharyngitis

Pregnancy & Lactation

  • Pregnancy Category C
  • Tofacitinib should be used during pregnancy only if potential benefit justifies potential risk to the fetus
  • Unknown excretion of Tofacitinib in human milk

Precautions & Warnings

  • Avoid use during active serious infection
  • Use with caution in patients at increased risk of gastrointestinal perforations
  • Recommended laboratory monitoring for potential changes in various blood parameters
  • Avoid use with live vaccines
  • Do not initiate if absolute lymphocyte count < 500 cells/mm3, absolute neutrophil count < 1000 cells/mm3, or hemoglobin < 9 gm/dL

Use in Special Populations

  • Safety and effectiveness not established in pediatric patients
  • Higher frequency of serious infection among Tofacitinib-treated subjects 65 years of age and older
