Indications
- Treatment of hypertension in adult patients
- Treatment of hypertension in pediatric patients 6 years of age and older
- Lowering blood pressure to reduce the risk of cardiovascular events
- Treatment of heart failure
- Reduction of mortality in hemodynamically stable patients within 24 hours of acute myocardial infarction
Pharmacology
- Competitively inhibits ACE
- Increases plasma renin activity
- Reduces aldosterone secretion
- Promotes vasodilation
- Reduces blood pressure
Dosage
- Initial adult dose: 10 mg/day
- Initial child dose for hypertension: 0.07 mg/kg
- Dosage adjustment for patients with renovascular hypertension, volume depletion
- Maintenance dose: up to 80 mg/day if needed
- Incremental dose adjustment for heart failure
Administration
May be taken with or without food
Interaction
- Enhances hypotensive effect with diuretics
- Increases risk of renal function deterioration and decreases antihypertensive effect with NSAIDs
- Increases serum levels and toxicity of lithium
Contraindications
- History of angioedema related to previous ACE inhibitor treatment
- Concomitant use with aliskiren in patients with diabetes or renal impairment
- Pregnancy
- Children with GFR <30 mL/min/1.73 m2

Precautions & Warnings
- Bilateral renal artery stenosis or a single kidney with unilateral renal artery stenosis
- Collagen vascular disease
- Acute MI at risk of further haemodynamic deterioration
- Aortic stenosis and hypertrophic cardiomyopathy
- Increased risk of angioedema in black patients
- Renal impairment
- Lactation
- Children under 6 years old
Use in Special Populations
- Adult CrCl 10-30 ml/min: Initially, 2.5-5 mg once daily
- Adult CrCl 31-80 ml/min: Initially 5-10 mg once daily
- Child: Do not give if GFR <30 mL/min/1.73 m2
