Medicine Details

Indications

  • Bronchial asthma
  • Bronchospasm
  • Chronic obstructive pulmonary disease (COPD)
  • Pulmonary disease with spastic bronchial component

Pharmacology

Doxophylline is a novel bronchodilator. It selectively inhibits phosphodiesterase 4, relaxes bronchial smooth muscle, and has decreased affinities toward adenosine A1 and A2 receptors.

Dosage & Administration

  • Elderly: 200 mg tablet two or three times daily
  • Adults: 400 mg tablet two or three times daily
  • Children:
    • >12 years of age: 10 ml syrup or 200 mg tablet two or three times daily
    • 6-12 years of age: 6-9 mg/kg body weight two times daily
  • Special Dosage: If required daily dose of Doxophylline is 400 mg, Doxophylline SR tablet to be taken once daily or as prescribed by the physician

Interaction

  • Not to be administered with other xanthine preparations
  • Limit consumption of caffeine-containing beverages and food
  • Caution with ephedrine or other sympathomimetic drugs
  • Concurrent administration with erythromycin, TAO, lincomycin, clindamycin, allopurinol, cimetidine, influenza vaccine, propanolol, phenytoin, and other anticonvulsants may affect hepatic clearance

Contraindications

  • Known hypersensitivity to the drug or other xanthine derivatives
  • Acute myocardial infarction
  • Hypotension
  • Lactation

  • Nausea
  • Vomiting
  • Epigastric pain
  • Headache
  • Irritability
  • Insomnia
  • Tachycardia
  • Tachypnea
  • Hyperglycemia (rare)
  • Albuminuria (rare)
  • Severe cardiac arrhythmias (in case of overdose)
  • Tonic-clonic seizure (in case of overdose)

Pregnancy & Lactation

  • Effects in Animals: No interference with pre and postnatal growth
  • Clinical Use in Pregnancy: Should be evaluated carefully case by case based on risk-benefit ratio
  • Lactation: Contraindicated
  • Effects on ability to drive and use machines: No interference

Precautions & Warnings

  • Factors may reduce hepatic clearance of xanthine derivatives
  • Caution in patients with cardiac disease, hypertension, elderly, severe hypoxemia, hyperthyroidism, chronic corpulmonale, congestive heart failure, liver disease, peptic ulcer, renal impairment
  • Monitoring of blood levels recommended in cases of factors influencing drug clearance
  • Caution in patients with congestive heart failure due to slow drug clearance

Overdose Effects

Severe cardiac arrhythmias and tonic-clonic seizure may occur
