Description

  • Esomeprazole MUPS tablet is indicated in various conditions like GERD, NSAID associated gastric ulcer, H. pylori eradication, Zollinger-Ellison syndrome, and idiopathic hypersecretion.
  • MUPS is the abbreviation for Multiple-Unit Pellet System, which combines the advantages of tablets and pellet-filled capsules in one dosage form. It ensures greater bioavailability, uniform drug release, once daily dosing, and better and more uniform drug absorption.
  • The tablets ensure rapid and uniform gastric emptying and subsequent uniform drug dissolution of pellets in the gastrointestinal tract.

Composition

  • Esomeprazole MUPS 20 Tablet contains 133.333 mg of Esomeprazole enteric coated pellets equivalent to 20 mg Esomeprazole.
  • Esomeprazole MUPS 40 Tablet contains 266.67 mg of Esomeprazole enteric coated pellets equivalent to 40 mg Esomeprazole.

Pharmacology

  • Esomeprazole inhibits gastric acid by blocking the hydrogen-potassium adenosine triphosphatase enzyme system (the proton pump) of the gastric parietal cell.
  • It is effective for short-term treatment of gastric and duodenal ulcers and for prevention and treatment of NSAID associated gastric ulcers.

Dosage

  • Adult dose for erosive esophagitis: 40 mg once daily for 4 weeks, and for maintenance of healing: 20 mg once daily.
  • For risk reduction in NSAID associated gastric ulcer: 20 mg once daily for 4-8 weeks.
  • For H. pylori eradication: 20 mg twice daily for 7 days, and for Zollinger-Ellison syndrome: 40-80 mg twice daily.
  • Dosages for children and adolescents are also specified based on age and conditions.

Administration

  • Esomeprazole MUPS tablets should be swallowed whole with liquid and should not be chewed or crushed. If required, the tablets can be dispersed in non-carbonated water.
  • The dispersed tablets must be consumed immediately or within 30 minutes, and no other liquids should be used.

Interaction

  • Extensively metabolized in the liver by CYP2C19 and CYP3A4.
  • No clinically relevant interactions with drugs metabolized by CYPs 1A2, 2A6, 2C9, 2D6, 2E1, and 3A4 would be expected.
  • May interfere with the absorption of drugs where gastric pH is important, and should be used with caution in combination therapy with certain drugs.

Contraindications

  • Contraindicated in patients with known hypersensitivity to any of the formulation.

SideEffects

  • Frequently occurring adverse events include headache, diarrhea, nausea, flatulence, abdominal pain, constipation, and dry mouth.
  • No difference in types of related adverse events seen during maintenance treatment up to 12 months compared to short-term treatment.

PregnancyAndLactation

  • The manufacturer advises caution. It is not known if Esomeprazole or its metabolites appear in human breast milk.

PrecautionsAndWarnings

  • Should be used carefully in patients with severe liver dysfunction and severe renal impairment.
  • May slightly increase the risk of hip, wrist, and spine fracture, particularly when taken for more than one year.
