Indications

  • Relief from chronic heartburn symptoms
  • Healing of erosive esophagitis
  • Maintenance of healing of erosive esophagitis
  • Eradication of Helicobacter pylori infection
  • Treatment of Zollinger-Ellison Syndrome
  • Management of acid-related Dyspepsia
  • Treatment of Duodenal & Gastric ulcer

Pharmacology

  • Proton pump inhibitor
  • Suppresses gastric acid secretion
  • Absorption through enteric-coated pellet formulation
  • Metabolized in the liver by cytochrome P450 enzyme system

Dosage

  • Healing of Erosive Esophagitis: 20 mg or 40 mg Once Daily for 4-8 Weeks
  • Symptomatic GERD: 20 mg Once Daily for 4 Weeks
  • H. pylori eradication: Triple Therapy with specific doses for 10 days
  • IV Injection dosage for adults and pediatric patients
  • Risk reduction of rebleeding of Gastric or Duodenal Ulcers

Administration

  • Tablet or capsule should be swallowed whole and taken one hour before a meal
  • Oral Suspension should be taken into a small glass containing 15 ml of water
  • IV Injection to be given over a period of at least 3 minutes

Interaction

  • Extensively metabolized in the liver by CYP2C19 and CYP3A4
  • May interfere with the absorption of certain drugs due to its effect on gastric pH
  • No clinically significant interactions with specific drugs

Contraindications

  • Contraindicated in patients with known hypersensitivity to any of the formulation

Side Effects

  • Frequently occurring adverse events include headache, diarrhea, nausea, flatulence, abdominal pain, constipation, dry mouth

Pregnancy & Lactation

  • No adequate and well-controlled studies in pregnant women
  • Animal studies have revealed no teratogenic effects
  • Breastfeeding should be discontinued if esomeprazole use is essential

Precautions & Warnings

  • Symptomatic response to therapy does not preclude the presence of gastric malignancy
  • Specific instructions for patients with difficulty swallowing capsules
  • Antacids may be used while taking Onepro

Use in Special Populations

  • Safety and effectiveness in pediatric patients have not been established
  • No overall differences in safety and efficacy observed between the elderly and younger individuals
  • No dosage adjustment recommended for patients with mild to moderate hepatic insufficiency

Overdose Effects

  • Major signs of acute toxicity in rats
  • No specific antidote known
  • Treatment should be symptomatic and supportive
