Pharmacology
Provides anti-inflammatory action by inhibiting Phospholipase A2 enzyme responsible for prostaglandin synthesis. Decreases release of certain chemicals important in the immune system, providing immunosuppressive action.
Dosage & Administration
- Adults:
- Up to 120 mg/day for acute disorders
- Maintenance doses in most conditions are within the range of 3-18 mg/day for adults
- Maintenance dose for rheumatoid arthritis: usually within the range of 3-18 mg/day
- High doses of 48-72 mg/day may be needed for bronchial asthma treatment during acute attacks
- Children:
- Indications for glucocorticoids are the same as for adults
- Usual maintenance dose for juvenile chronic arthritis: between 0.25-1.0 mg/kg/day
- Initial dose of usually 1.5 mg/kg/day for nephrotic syndrome followed by down titration according to clinical need
- Withdrawal: In patients who have received more than physiological doses of systemic corticosteroids, withdrawal should not be abrupt
Interaction
Metabolized in the liver. Maintenance dose may need to be increased if co-administered with liver enzyme inducers. Maintenance dose may be reduced if co-administered with liver enzyme inhibitors.
Contraindications
Hypersensitivity to any of the ingredients. Patients receiving live virus immunization.

Pregnancy & Lactation
- Pregnancy: Corticosteroids may increase the risk of intrauterine growth retardation. Should only be prescribed when benefits outweigh the risks.
- Nursing Mother: Doses of up to 50 mg daily are unlikely to cause systemic effects in the infant. Infants of mothers taking higher doses may have a degree of adrenal suppression.
Precautions & Warnings
- Special caution required for patients with certain clinical conditions
- Frequent patient monitoring is necessary
- May cause salt and water retention and increased excretion of potassium in certain conditions
- Special caution needed for patients with gastritis, oesophagitis, diverticulitis, ulcerative colitis, diabetes mellitus, osteoporosis, myasthenia gravis, renal insufficiency, emotional instability, psychotic tendency, epilepsy, liver failure, hypothyroidism, cirrhosis, or ocular herpes simplex
Use in Special Populations
- Hepatic Impairment: Blood levels may be increased, careful monitoring and adjustment of the dose needed
- Renal Impairment: No special precautions necessary
- Elderly: No special precautions necessary, but common adverse effects may have more serious consequences in old age
