Indications

  • Management of moderate to severe acute post-operative pain
  • Relief of pain, inflammation, and light sensitivity after ocular surgery
  • Decreasing eye inflammation or redness caused by seasonal allergies

Description

  • Belongs to pyrrolo-pyrrole group of nonsteroidal anti-inflammatory drug (NSAID)
  • Chemical name: 5 benzoyle-2,3-dihydro-1H-pyrroligine-1-carboxylic acid, compound with 2 - amino-2-(hydroxymethyl)-1,3-propanediol (1:1)
  • Inhibits synthesis of prostaglandins as a peripherally acting analgesic
  • Biologically active form: S-form
  • Linear pharmacokinetic property
  • Highly protein bound
  • Metabolized in the liver
  • Metabolites and some unchanged drugs excreted in urine

Pharmacology

  • Potent analgesic of the non-steroidal anti-inflammatory drugs (NSAIDs)
  • Inhibits the cyclooxygenase enzyme system
  • Minimal anti-inflammatory effect at analgesic dose

Dosage & Administration

  • Tablet: Recommended dose is 10 mg every 4-6 hours for short-term use (up to 7 days)
  • Tablet: Not recommended for chronic use or doses exceeding 40 mg/day
  • Injection: Used for moderately severe, acute pain management, usually postoperative
  • Injection: Administered as single or multiple doses
  • Injection: IV bolus given over no less than 15 seconds, IM administration given slowly and deeply into muscle
  • Eye Drops: Adults - 1 drop in each eye 4 times daily

Interaction

  • Other NSAIDs or aspirin increases side effects of Etorac
  • Anti-coagulants enhance anti-coagulant effect
  • Beta Blocker reduces anti-hypertensive effect
  • ACE Inhibitors increase risk of renal impairment
  • Methotrexate enhances toxicity of methotrexate

Contraindications

  • Hypersensitivity to Ketorolac or other NSAIDs
  • Use not recommended in children under 16 years
  • Contraindicated as prophylactic analgesic before surgery

Precautions & Warnings

  • Caution in patients with active or suspected peptic ulcer, gastrointestinal bleeding, asthma, or liver dysfunction
  • For Eye Drops: Discontinue use with evidence of corneal epithelial breakdown, monitor closely for corneal health, do not use while wearing contact lenses
