Indications
- Adjunct to diet and exercise in type 2 diabetes mellitus patients
- Adequate glycemic control
- Combination therapy of glimepiride and metformin
Pharmacology
- Sulfonylurea antidiabetic agent
- Decreases blood glucose concentration
- Stimulates the release of insulin
- Improves insulin sensitivity
- Reduces basal hepatic glucose production
- Increases peripheral tissue sensitivity to insulin
Dosage
- Governing by desired blood glucose level
- Lowest dosage sufficient for metabolic control
- Regular blood and urine glucose level measurement
- Maximum recommended dose: 8 mg glimepiride, 2000 mg metformin
Administration
- Swallowed whole, not crushed or chewed
Interaction
- Metabolized by cytochrome P450 2C9 (CYP2C9)
- Potentiation of blood-glucose-lowering effect with concomitant drugs
- Weakening of blood-glucose-lowering effect with concomitant drugs
- Alcohol intake may potentiate or weaken action
Contraindications
- Hypersensitivity to glimepiride, metformin, sulfonylureas, sulfonamides
- Pregnant and breastfeeding women
- Severe impairment of liver function
- Severe renal failure or dysfunction

- Hypoglycemia
- Visual impairment
- Gastrointestinal symptoms
- Hepatitis and elevation of liver enzyme levels
- Alopecia
- Allergic or pseudo allergic reactions
Pregnancy & Lactation
- Glimepiride must not be taken during pregnancy
- Metformin should not be used during pregnancy
- Glimepiride must not be taken by breastfeeding women
- Metformin excreted into milk, decision should be made whether to discontinue nursing
Precautions & Warnings
- Increased risk of hypoglycemia in initial weeks of treatment
- Caution in patients with G6PD-deficiency
- Regular monitoring of thyroid-stimulating hormone levels
Use in Special Populations
- Insufficient data for pediatric use
- Assessment of GFR before initiation of treatment with metformin
- Renal function assessment in elderly and at increased risk
Overdose Effects
- Severe life-threatening hypoglycemia
- Lactic acidosis
- Pancreatitis may occur in the context of a metformin overdose
