Indications
- Prophylaxis of organ rejection in kidney transplant in adults
- Prophylaxis of organ rejection in pediatric kidney transplant patients aged 5 and older
Pharmacology
- Mycophenolic acid delayed-release tablets are an enteric coated formulation of mycophenolate sodium
- Immunosuppressive agent
- Inhibitor of inosine monophosphate dehydrogenase (IMPDH)
- Cytostatic effects on lymphocytes
- Prevents acute rejection of kidney and heart allotransplantation
- Decreases antibody production
Dosage & Administration
- Recommended dose for adults: 720 mg twice daily
- Recommended dose for pediatric patients at least 6 months post-transplant: 400 mg/m2 twice daily
- Different dosing for pediatric patients based on body surface area
- Tablets should be taken on an empty stomach, 1 hour before or 2 hours after food intake
- Tablets should not be crushed, chewed, or cut prior to ingesting
Interaction
- Caution with concomitant administration of antacids, azathioprine, cyclosporin, rifampin, sevelamer, cholestyramine, acyclovir, metronidazole, hormonal contraceptives
Contraindications
- Not to be used interchangeably with mycophenolate mofetil tablets and capsules without physician supervision
Side Effects
- Common adverse reactions include anemia, leukopenia, constipation, diarrhea, vomiting, dyspepsia, urinary tract infection, CMV infection, insomnia, postoperative pain
Pregnancy & Lactation
- Pregnancy Category D - can cause fetal harm
- Discontinue Mycophenolic acid during lactation or discontinue nursing within 6 weeks after stopping therapy
- Consider the importance of the drug to the mother
Precautions & Warnings
- Caution due to embryofetal toxicity
- Increased risk of new or reactive viral infections
- Risk of blood dyscrasias including pure red cell aplasia
- Risk of serious GI tract complications
- May increase the risk of developing lymphomas and other malignancies, particularly of the skin
- Avoid use of live vaccine during treatment
