Indications
- treatment of insomnia characterized by difficulty with sleep maintenance
- clinical trials up to 3 months duration
Pharmacology
- Doxepin binds with high affinity to the histamine H1 receptor
- antagonist of histamine H1 receptor
Dosage & Administration
- recommended dose of Doxepin for adults is 6 mg once daily
- 3 mg once daily dose for some patients
- starting dose of 3 mg in elderly patients
- should be taken within 30 minutes of bedtime
- not to be taken within 3 hours of a meal
- total dose should not exceed 6 mg per day
- safety and effectiveness not evaluated in pediatric patients
Interaction
- metabolized by hepatic cytochrome P450 isozymes
- exposure doubled with cimetidine co-administration
- potentiated sedative effects with alcohol, CNS depressants, and sedating antihistamines
- reported case of severe hypoglycemia with tolazamide co-administration
Contraindications
- contraindicated in individuals with hypersensitivity to doxepin HCl or its inactive ingredients
- not to be administered with Monoamine Oxidase Inhibitors (MAOIs)
- contraindicated in individuals with untreated narrow-angle glaucoma or severe urinary retention
Side Effects
- abnormal thinking and behavioral changes
- suicide risk and worsening of depression
- CNS Depressant effects
Pregnancy & Lactation
- Pregnancy Category C
- no adequate and well-controlled studies in pregnant women
- excreted in human milk after oral administration
- caution advised when administered to nursing women
Precautions & Warnings
- need to evaluate for comorbid diagnoses before starting treatment
- risk of abnormal thinking and behavioral changes
- risk of suicide and worsening of depression
- discontinuation considered for patients reporting sleep-driving episodes
