Indications
- Treatment of insomnia characterized by difficulty with sleep maintenance
Pharmacology
- Binds with high affinity to the histamine H1 receptor
- Functions as an antagonist of the H1 receptor
Dosage & Administration
- Recommended dose for adults: 6 mg once daily
- Recommended starting dose for elderly: 3 mg once daily
- Should be taken within 30 minutes of bedtime
- Should not be taken within 3 hours of a meal
- Total dose should not exceed 6 mg per day
Interactions
- Metabolized by hepatic cytochrome P450 isozymes CYP2C19 and CYP2D6
- Exposure doubled with concomitant administration of cimetidine
- Potentiated sedative effects with alcohol, sedating antihistamines, and CNS depressants
- Reported case of severe hypoglycemia when used with tolazamide
Contraindications
- Contraindicated in individuals with hypersensitivity to doxepin HCl or its inactive ingredients, or other dibenoxepines
- Contraindicated with concomitant use of MAO inhibitors
- Contraindicated in individuals with untreated narrow-angle glaucoma or severe urinary retention
Side Effects
- Abnormal thinking and behavioral changes
- Suicide risk and worsening of depression
- CNS depressant effects
Pregnancy & Lactation
- Pregnancy Category C
- Excreted in human milk after oral administration, caution in nursing women
Precautions & Warnings
- Need to evaluate for comorbid diagnoses before initiating treatment
- Complex behaviors such as 'sleep-driving' reported
- Suicide risk and worsening of depression in primarily depressed patients
- Risk of abnormal behaviors in patients not fully awake after taking the medicine
