Indications
- Treatment of essential hypertension in adults
- Reduction of cardiovascular morbidity in adults with atherothrombotic cardiovascular disease
- Reduction of cardiovascular morbidity in adults with type 2 diabetes mellitus
- Reduction of cardiovascular morbidity in adults with documented target organ damage
Pharmacology
- Non-peptide angiotensin-ll receptor antagonist
- Blocks vasoconstrictor and aldosterone-secreting effects of angiotensin-ll
- Greater affinity for AT1 receptor than for AT2 receptor
- Does not affect the response to bradykinin
- Does not bind to or block other hormone receptors or ion channels
Dosage & Administration
- Usual starting dose of 40 mg once a day
- Dose-related blood pressure response
- Most antihypertensive effect within 2 weeks
- Maximal reduction generally attained after 4 weeks
- No initial dosage adjustment for elderly patients or those with renal impairment
- Not recommended for patients ≥75 years old or with hepatic impairment
Interaction
- Increased risk of renal impairment and loss of antihypertensive effect with NSAIDS
- Co-administration with aliskiren in patients with diabetes should be avoided
Contraindications
- Known hypersensitivity to the product or any of its components
Side Effects
- Common side effects: sinus pain, congestion, back pain, diarrhea
- Common side effects for cardiovascular risk reduction: intermittent claudication, skin ulcer
Pregnancy & Lactation
- Pregnancy Category C (first trimester) and D (second and third trimester)
- Consider adverse effects on the nursing infant when deciding whether to discontinue nursing or the drug
Precautions & Warnings
- Avoid fetal or neonatal exposure
- Monitor carefully in patients with impaired hepatic or renal function
- Avoid concomitant use of an ACE inhibitor and angiotensin receptor blocker
Overdose Effects
- Manifestation of overdosage: hypotension, dizziness, tachycardia
- Possible increase in serum creatinine and acute renal failure
