Indications
- Moderate to severe painful conditions
- Postoperative pain
- Colic and spastic pain
- Cancer pain
- Joint pain
- Neck and back pain
- Pain associated with osteoporosis
Pharmacology
- Centrally acting synthetic analgesic compound
- Inhibits reuptake of neurotransmitters - serotonin and noradrenaline
- Modifies transmission of pain impulses
- Activates serotonergic and noradrenergic pathways
- Stimulation of mu-opioid receptors
- Modulation of central monoaminergic inhibitory pathways
Dosage & Administration
- Usual doses of 50 to 100 mg every 4 to 6 hours
- Initial dose of 100 mg for acute pain
- Initial dose of 50 mg for chronic painful conditions
- Sustained release: One SR capsule or tablet every 12 hours
- Injection: 50-100 mg every 4 to 6 hours
- Suppository: 100 mg Tramadol Hydrochloride 6 hourly for adults
- Total daily dosage by mouth should not exceed 400 mg
Interaction
- MAO inhibitors not recommended for combination
- Concomitant administration of carbamazepine increases metabolism and requires adjusting dose

- Hypersensitivity to the drug
- Acute intoxication with certain substances

Pregnancy & Lactation
- Safe use in pregnancy not established
- Crosses the placenta
- Use during pregnancy justified if benefit outweighs risk to the fetus
- Not to be administered during breastfeeding as it has been detected in breast milk
Precautions & Warnings
- Respiratory depression risk
- Not recommended for opioid-dependent patients
- Caution when used with CNS depressants
- Caution with concomitant MAO inhibitors
- Caution in patients with increased intracranial pressure or head injury, and acute abdominal conditions
Use in Special Populations
- Children from the age of 1 year can be given Winpain in a specific dose
- Suppository not suitable for children and adolescents below the age of 14 years
- SR Capsules not studied in children, safety and efficacy not established
