Indications

Pharmacology
- ACE inhibitor
- Blocks conversion of angiotensin I to angiotensin II
- Decreases vasopressor activity
- Decreases aldosterone secretion

Dosage
- Initial dose of 1.25-2.5 mg once daily for hypertension
- Maintenance dosage of 2.5-20 mg daily
- Initial dose of 2.5 mg twice daily for heart failure after myocardial infarction
- Titration to a target daily dose of 5 mg twice daily
Administration
- Swallow tablets with sufficient liquid
- Do not chew or crush tablets
- Can be taken before, during, or after a meal
Interactions
- Concomitant administration with diuretics
- Concomitant therapy with lithium
- Alcohol may exacerbate effects on driving ability
- NSAIDs may reduce antihypertensive effect
Contraindications
- Hypersensitivity to ramipril or any other ACE inhibitor
- History of angioedema
- Concomitant sacubitril/valsartan therapy
- Relevant renal artery stenosis
- Hypotensive or haemodynamically unstable states
- Concomitant aliskiren-containing medicines
- Concomitant angiotensin II receptor antagonists
- During pregnancy

- Use with caution in patients with impaired renal function
- Use with caution in patients with hyperkalemia
- Use with caution in patients with hypotension
- Use with caution in patients with impaired hepatic function
Use in Special Populations
- Elderly patients: consider a reduced initial dose
- Hepatic impairment: initiate treatment under close medical supervision
- Renal impairment: consider a reduced initial dose
- Consideration for patients pretreated with a diuretic
Overdose Effects
- Excessive peripheral vasodilatation
- Marked hypotension
- Shock
- Bradycardia
- Electrolyte disturbances
- Renal failure
