Description
Potent anti-inflammatory steroid with greater anti-inflammatory potency than Prednisolone and less tendency to induce sodium and water retention.
Pharmacology
- Pharmacodynamic Properties: Potent anti-inflammatory agent with the capacity to profoundly inhibit the immune system
- Pharmacokinetic Properties: High absolute bioavailability, rapid absorption, widely distributed into tissues, metabolized in the liver
Dosage & Administration
- Usual range of 2-48 mg daily in divided doses
- Initial dosage varies from 4-48 mg per day depending on the specific disease entity being treated
- Maintenance dosage determined by decreasing the initial drug dosage in small decrements at appropriate time intervals
Interaction
- Inhibition of metabolism by Erythromycin, Clarithromycin, Phenobarbital, Phenytoin, Rifampin, and Ketoconazole
- Increased effect by estrogens, including birth control pills
- Reduced metabolism with Cyclosporin and Methigic
- Potential increase or decrease in the effect of blood thinners
Contraindications
Systemic fungal infections and known hypersensitivity to components
Side Effects
Range from fluid retention to serious irreversible bodily damage, including potential corticosteroid insufficiency symptoms upon discontinuation
Pregnancy & Lactation
Pregnancy category C; use in nursing mothers not adequately evaluated
Precautions & Warnings
- Adrenocortical insufficiency may persist for months after discontinuation of therapy
- Enhanced effect on patients with hypothyroidism and in those with cirrhosis
- Cautious use in patients with ocular herpes simplex and hypoprothrombinemia
- Careful observation of growth and development of infants and children on prolonged therapy

Rare occurrences with no specific antidote available; treatment is supportive and symptomatic
