Description
- Potent anti-inflammatory steroid
- Greater anti-inflammatory potency than Prednisolone
- Less tendency to induce sodium and water retention
Pharmacology
- Potent anti-inflammatory agent
- Inhibits the immune system
- Influences kidney and fluid & electrolyte balance
- Affects lipid, protein, and carbohydrate metabolism
- Affects skeletal muscle, cardiovascular system, immune system, nervous system, and endocrine system
- High absolute bioavailability (82% to 89%) following oral administration
- Rapidly absorbed
- Widely distributed into tissues
- Crosses the blood-brain barrier and placental barrier
- Secreted in breast milk
- Metabolized in the liver to inactive metabolites
- Haemodializable
- Plasma protein binding approximately 77%
Dosage & Administration
- Usual range is 2-48 mg daily in divided doses
- Initial dosage may vary from 4-48 mg per day depending on specific disease entity
- Maintenance dosage determined by decreasing initial drug dosage
- Recommended gradual withdrawal if the drug is to be stopped
Interaction
- Erythromycin, Clarithromycin, Phenobarbital, Phenytoin, Rifampin, and Ketoconazole inhibit the metabolism of Methigic
- Estrogens, including birth control pills, can increase the effect of corticosteroids by 50%
- Cyclosporin reduces the metabolism of Methigic
- Methigic may increase or decrease the effect of blood thinners (e.g. Warfarin)
- Dose adjustments may be necessary for interactions
Contraindications
- Systemic fungal infections
- Known hypersensitivity to components
Side Effects
- Short courses are usually well-tolerated with few, mild side effects
- Long term, high doses may produce serious side effects
- Lowest effective doses should be used for the shortest length of time
- Alternate day dosing can help reduce side effects
- Range of side effects from mild annoyances to serious irreversible bodily damage
Pregnancy & Lactation
- Pregnancy category C
- Should be given only if potential benefit justifies potential risk to the fetus
- Not adequately evaluated in nursing mothers
Precautions & Warnings
- Adrenocortical insufficiency may persist for months after discontinuation of therapy
- Mineralocorticoid secretion may be impaired
- Enhanced effect on patients with hypothyroidism and in those with cirrhosis
- Cautiously used in patients with ocular herpes simplex
- Careful observation of growth and development in infants and children on prolonged therapy
Overdose Effects
- Report of acute toxicity and/or death following an overdose are rare
- No specific antidote available
- Treatment is supportive and symptomatic
- Serum electrolytes should be monitored
