Indications

Pharmacology
- Angiotensin converting enzyme (ACE) inhibitor
- Blocks conversion of angiotensin I to angiotensin II
- Decreases vasopressor activity
- Reduces aldosterone secretion
Dosage
- Initial dose range: 1.25-2.5 mg once daily
- Maintenance dosage range: 2.5-20 mg daily
- Dosage adjustment at 2 week intervals
- Renal impairment dosage adjustment
Administration
- Tablets must be swallowed whole
- Absorption not significantly affected by food
- Can be taken before, during, or after a meal
Interaction
- Contraindicated with diuretics
- Potential interaction with lithium
- Risk of reduced BP with alcohol
- NSAIDs may reduce antihypertensive effect
Contraindications
- Hypersensitivity to ramipril or other ACE inhibitors
- History of angioedema
- Concomitant sacubitril/valsartan therapy
- Haemodynamically relevant renal artery stenosis
- Hypotensive or haemodynamically unstable states
- Concomitant use with aliskiren-containing medicines
- Concomitant use with angiotensin II receptor antagonists (AIIRAs)
Side Effects
- Common: dizziness, headache, fatigue, asthenia
- Less frequent: hypotension, cough, nausea, rash, oliguria
- Rare: angioneurotic oedema, anaphylactic reactions, hyperkalaemia
Pregnancy & Lactation
- Contraindicated during pregnancy
- Avoid pregnancy during ACE inhibitor treatment
- Discontinue ACE inhibitor if intending to become pregnant
Precautions & Warnings
- Caution in impaired renal function, hyperkalaemia, hypotension, impaired hepatic function
Use in Special Populations
- Elderly: consider reduced initial dose
- Hepatic impairment: initiate under close medical supervision
- Renal impairment: dosage adjustment based on creatinine clearance
Overdose Effects
- Signs: hypotension, bradycardia, electrolyte disturbances
- Management: detoxification, administration of adrenergic agonists
