Technical Specifications
- Pharmacology: Ramipril is an angiotensin converting enzyme (ACE) inhibitor, which blocks the conversion of angiotensin I to angiotensin II, resulting in decreased vasopressor activity and decreased aldosterone secretion. It is long-acting and well-tolerated.
- Dosage: Dosage must be adjusted according to patient tolerance and response. Initial dose for hypertension: 1.25-2.5 mg once daily. Initial dose for heart failure after myocardial infarction: 2.5 mg twice daily. Dosage in renal impairment should be titrated based on individual tolerance and BP response.
- Administration: Tablets must be swallowed whole with sufficient liquid and should not be chewed or crushed. Absorption is not significantly affected by food.
- Interaction: Concomitant administration with diuretics may lead to serious hypotension. NSAIDs may reduce the antihypertensive effect of Acecard and cause deterioration of renal function.
- Contraindications: Must not be used in patients with hypersensitivity to ramipril, history of angioedema, during pregnancy, and with certain comorbidities.
- Side Effects: Common side effects include dizziness, headache, fatigue, and asthenia. Rare side effects include angioneurotic edema, anaphylactic reactions, and hyperkalemia.
- Pregnancy & Lactation: Must not be taken during pregnancy. Pregnancy should be avoided during treatment. Not recommended during breastfeeding.
- Precautions & Warnings: Use with caution in patients with impaired renal function, hyperkalemia, hypotension, and impaired hepatic function.
- Use in Special Populations: Elderly patients may require a reduced initial dose. Close medical supervision is necessary for patients with hepatic or renal impairment.
