Indications

Pharmacology

  • ACE inhibitor
  • Blocks conversion of angiotensin I to angiotensin II
  • Decreases vasopressor activity
  • Decreases aldosterone secretion
  • Long-acting
  • Well-tolerated

Dosage

  • Initial dose 1.25-2.5 mg for hypertension
  • Dosage adjustment at 2-week intervals
  • Maintenance dosage 2.5-20 mg daily
  • 5 mg twice daily for congestive heart failure after myocardial infarction
  • Dosage titration to 10 mg for prevention of major cardiovascular events
  • Different dosage for renal impairment

Administration

  • Tablets to be swallowed with liquid
  • Must not be chewed or crushed
  • Absorption not affected by food

Interaction

  • Concomitant administration with diuretics
  • Concomitant therapy with lithium
  • Effect of alcohol on blood pressure
  • NSAIDs reducing antihypertensive effect

Contraindications

  • Hypersensitivity to ramipril or ACE inhibitors
  • History of angioedema
  • Concomitant use with sacubitril/valsartan therapy
  • Haemodynamically relevant renal artery stenosis
  • Hypotensive or haemodynamically unstable states
  • Concomitant use with aliskiren-containing medicines or AIIRAs
  • During pregnancy
  • Concomitant use with extracorporeal treatments

Pregnancy & Lactation

  • Not recommended during breastfeeding

Precautions & Warnings

  • Use with caution in impaired renal function
  • Use with caution in hyperkalaemia
  • Use with caution in hypotension
  • Use with caution in impaired hepatic function

Use in Special Populations

  • Reduced initial dose for Elderly
  • Caution in Hepatic impairment
  • Caution in Renal impairment
  • Consideration for patients pretreated with a diuretic

Overdose Effects

  • Excessive peripheral vasodilatation
  • Marked hypotension
  • Shock
  • Bradycardia
  • Electrolyte disturbances
  • Renal failure
