Description
- Piramil is indicated for the treatment of hypertension, congestive heart failure, post-myocardial infarction treatment, diabetic or non-diabetic nephropathy, and reducing the risk of myocardial infarction, stroke, or cardiovascular death in high-risk patients.

Dosage Instructions
- Dosage must be adjusted according to patient tolerance and response. Initial doses range from 1.25-2.5 mg for hypertension and higher for other indications. Dosage generally adjusted no more rapidly than at 2-week intervals.
Dosage in Special Populations
- Elderly: Consider reduced initial dose. Hepatic Impairment: Initiate treatment under close medical supervision. Renal Impairment: Adjust initial dose based on creatinine clearance. Patients pretreated with a diuretic: Consider discontinuing or reducing the diuretic dose before starting Piramil.
Administration Instructions
- Tablets must be swallowed with sufficient liquid and not chewed or crushed. Absorption is not significantly affected by food, so can be taken before, during, or after a meal.
Interaction Warnings
- Concomitant administration with diuretics may lead to serious hypotension. Concomitant therapy with lithium may increase serum lithium concentration. NSAIDs may reduce the antihypertensive effect and cause renal function deterioration.
Contraindications
- Hypersensitivity to ramipril or any other ACE inhibitor, history of angioedema, concomitant sacubitril/valsartan therapy, haemodynamically relevant renal artery stenosis, hypotensive or haemodynamically unstable states, pregnancy, concomitant use with AIIRAs and extracorporeal treatments with negatively charged surfaces.
Side Effects
- Common side effects include dizziness, headache, fatigue, asthenia. Less frequent side effects include cough, nausea, vomiting, rash, urticaria, oliguria, anxiety, amnesia. Rarely-reported side effects: angioneurotic edema, anaphylactic reactions, hyperkalemia.
Pregnancy & Lactation Precautions
- Piramil must not be taken during pregnancy, should be avoided where indispensable, and discontinued if the patient intends to become pregnant. Not recommended during breastfeeding.
Precautions & Warnings
- Piramil should be used with caution in patients with impaired renal function, hyperkalemia, hypotension, and impaired hepatic function.
Overdose Effects
- Overdosage may cause excessive peripheral vasodilatation, bradycardia, electrolyte disturbances, renal failure. Management may involve gastric lavage, administration of adsorbents, and other supportive measures.

Storage Conditions
- Store at 30°C or below, protect from light, keep out of the reach of children, and do not use later than the date of expiry.

Pharmacology
- Ramipril is an ACE inhibitor that blocks the conversion of angiotensin I to angiotensin II, leading to decreased vasopressor activity and aldosterone secretion, exerting its antihypertensive activity.
