Indications
- Hypertension
- Congestive heart failure
- Acute myocardial infarction
- Glomerular or incipient nephropathy
- Reduction in the risk of myocardial infarction

- Increased cardiovascular risk
Pharmacology
- Angiotensin converting enzyme (ACE) inhibitor
- Blocks the conversion of angiotensin I to angiotensin II
- Decreased vasopressor activity
- Decreased aldosterone secretion
- Antihypertensive activity
- Management of heart failure
- Reduction of the risk of stroke and myocardial infarction
- Long acting and well tolerated
Dosage
- Adjusted according to patient tolerance and response
- Usual initial dose of 1.25-2.5 mg once daily for hypertension
- Initial dose of 2.5 mg twice daily for congestive heart failure after myocardial infarction
- Recommended dose of 2.5 mg once daily for prevention of major cardiovascular events
Administration
- Tablets to be swallowed with sufficient liquid
- Must not be chewed or crushed
- Absorption not significantly affected by food
- Can be taken before, during or after a meal
Interaction
- Concomitant administration with diuretics may lead to serious hypotension
- Concomitant therapy with lithium may increase serum lithium concentration
- Reduction in BP may affect the ability to drive and operate machinery
- NSAIDs may reduce the antihypertensive effect and cause deterioration of renal function
Contraindications
- Hypersensitivity to ramipril or any other ACE inhibitor or excipients
- History of angioedema
- Concomitant use with sacubitril/valsartan therapy
- Haemodynamically relevant renal artery stenosis
- Hypotensive or haemodynamically unstable states
- Concomitant use with certain medicines in patients with diabetes or moderate to severe renal impairment
- During pregnancy
- With angiotensin II receptor antagonists (AIIRAs) in patients with diabetic nephropathy
- Concomitant use with extracorporeal treatments leading to contact of blood with negatively charged surfaces

Pregnancy & Lactation
- Must not be taken during pregnancy
- Pregnancy must be excluded before starting treatment
- Pregnancy must be avoided in cases where treatment with ACE inhibitors is indispensable
- Not recommended during breastfeeding
Precautions & Warnings
- Use with caution in patients with impaired renal function
- Use with caution in patients with hyperkalaemia
- Use with caution in patients with hypotension
- Use with caution in patients with impaired hepatic function
Use in Special Populations
- Elderly: Consider reduced initial dose
- Hepatic impairment: Initiate treatment under close medical supervision
- Renal impairment: Adjust dosage based on creatinine clearance
- Patients pretreated with a diuretic: Consider discontinuing or reducing diuretic dose before starting treatment
Overdose Effects
- Excessive peripheral vasodilatation
- Marked hypotension
- Bradycardia
- Electrolyte disturbances
- Renal failure
