Indications
- spasticity resulting from multiple sclerosis
- flexor spasms and concomitant pain, clonus and muscular rigidity
- skeletal muscle spasm resulting from rheumatic disorders
- spinal cord injuries and other spinal cord diseases
- cerebrovascular accidents or neoplastic or degenerative brain disease
Pharmacology
- inhibits both monosynaptic and polysynaptic reflexes at the spinal level by stimulating the GABAB receptors
- inhibits the release of glutamate and aspartate
- antinociceptive effect
- CNS depression
Dosage & Administration
- 5 mg 3 times daily for adults & children over 10 years
- preferably with or after food
- gradually increased
- max. 100 mg daily
- dosage varies for children under 10 years based on age
Interaction
- Increased sedation with central nervous system agents
- risk of respiratory depression
- increased fall in blood pressure with antihypertensives
- potentiated pharmacological effects with tricyclic antidepressants
- reports of mental confusion, hallucinations, headaches, nausea and agitation with Parkinson's medication
- increased CNS depressant effects with MAO inhibitors
- consideration needed when administering with magnesium sulphate or other neuromuscular blocking agents
Contraindications
- hypersensitivity to any component of the product
Side Effects
- transient drowsiness
- daytime sedation
- dizziness
- weakness
- fatigue
- adverse reactions include effects on central nervous system, cardiovascular, gastrointestinal, genitourinary, and other systems
Pregnancy & Lactation
- Pregnancy category B3
- safe use not established during pregnancy
- administration should be based on physician's judgement
- excreted in breast milk but quantities expected to have no undesirable effects on infant
Precautions & Warnings
- lower doses for patients with impaired renal function
- cautious treatment for patients with psychotic disorders, depression, schizophrenia, or confusional states
- monitoring required for patients with epilepsy and muscle spasticity
- caution for patients with peptic ulcers, cerebrovascular diseases, or hepatic, renal or respiratory failure
- respiratory and cardiovascular function monitoring especially for patients with cardiopulmonary disease and respiratory muscle weakness
- neurogenic disturbances affecting bladder emptying may improve, but acute retention of urine may occur
- poor tolerance and lack of significant benefit for patients with stroke
- appropriate periodic laboratory tests for patients with hepatic diseases or diabetes mellitus
Overdose Effects
- gastric lavage in case of severe overdose
