Description
- Dopamine antagonist medication
- Treats nausea and vomiting
- Gastroprokinetic action

- Restores normal motility and tone of the upper gastrointestinal tract
- Enhances antral and duodenal peristalsis
- Facilitates gastric emptying
- Increases esophageal peristalsis and lower esophageal sphincter pressure
Dosage & Administration
- Should be taken 15-30 minutes before meals
- Adults: 10-20 mg every 6-8 hours daily
- Children: 0.2-0.4 mg/kg body weight every 6-8 hours daily
- Maximum dose of 80 mg daily
- Maximum period of treatment for acute nausea and vomiting is 12 weeks
Interaction
- Antagonizes the antidyspeptic effect of anticholinergic drugs
- Should not be given simultaneously with antacids and antisecretory drugs
- Metabolized through CYP3A4 pathway
- Inhibitors include azole antifungals, macrolide antibiotics, HIV protease inhibitors, nefazodone, etc.
- May influence the absorption of orally administered drugs
- Does not influence the blood levels of digoxin or paracetamol
- Does not potentiate the action of neuroleptics
- Suppresses unwanted peripheral effects of dopaminergic agonists
Contraindications
- Known hypersensitivity to the drug
- Neonates
- Gastrointestinal hemorrhage
- Mechanical obstruction or perforation
- Prolactin releasing pituitary tumor
Side Effects
- Transient intestinal cramps
- Rare extrapyramidal phenomena in young children
- Rare increase in plasma prolactin level
- Rare allergic reactions such as rash and urticaria
Pregnancy & Lactation
- Should be used with caution during the first trimester of pregnancy
- No evidence of increase in the risk of malformations in humans
- Excreted in breast milk
- Domperidone concentrations in breast milk are 4 times lower than corresponding plasma concentrations
- Not recommended for nursing mothers unless expected benefits outweigh potential risk
Precautions & Warnings
- Absolute caution in children
- Increased risk of extra-pyramidal reactions in young children
- Highly metabolized in the liver
- Caution in patients with hepatic impairment
- Use with caution in patients with severe renal insufficiency
- Patients on prolonged therapy should be reviewed regularly
Use in Special Populations
- Should only be given to infants with great caution and under close medical supervision

Overdose Effects
- Symptoms include drowsiness, disorientation and extrapyramidal reactions
- Activated charcoal and close observation recommended in case of overdosage
