Doxophylline is used to treat indications such as bronchial asthma, bronchospasm, chronic obstructive pulmonary disease (COPD), and pulmonary disease with spastic bronchial component.
Doxophylline has a novel bronchodilating mechanism of action by selectively inhibiting phosphodiesterase 4, relaxing bronchial smooth muscle, and decreasing affinities toward adenosine A1 and A2 receptors for a better safety profile.
Dosage and administration vary for elderly, adults, and children based on tablet or syrup forms and body weight considerations.
It's contraindicated in individuals with known hypersensitivity to the drug or other xanthine derivatives, patients with acute myocardial infarction, hypotension, and during lactation.
Possible side effects may include nausea, vomiting, epigastric pain, headache, insomnia, tachycardia, and in rare cases hyperglycemia or albuminuria, with severe cardiac arrhythmias and tonic-clonic seizures in case of overdose.
Use during pregnancy and lactation requires careful evaluation, and the product should be stored in a dry place away from light and heat, out of the reach of children, and used only on prescription of a specialist physician.

Elderly: 200 mg tablet two or three times daily
Adults: 400 mg tablet two or three times daily
Children:
- >12 years of age: 10 ml syrup or 200 mg tablet two or three times daily
- 6-12 years of age: 6-9 mg/kg body weight two times daily
Special release tablet: Doxophylline SR tablet to be taken once daily if required daily dose is 400 mg

Should not be administered with: Other xanthine preparations
Recommended to limit consumption of: Beverages and food containing caffeine
Caution with:
- Ephedrine or other sympathomimetic drugs
- Concurrent administration of erythromycin, TAO, lincomycin, clindamycin, allopurinol, cimetidine, influenza vaccine, and propranolol
- Phenytoin, other anticonvulsants, and cigarette smoking

Animal tests results: No interference with pre and postnatal growth
Clinical evidence and use during pregnancy: Should be evaluated carefully on a case-by-case basis
Contraindicated during lactation: 1
Effects on ability to drive and use machines: Does not affect patient's alertness

Factors reducing hepatic clearance: Age, congestive cardiac decompensation, chronic obstructive pulmonary disease, severe liver disease, concomitant infections
Monitoring recommendations: Blood levels of the drug for the control of the therapeutic range
