Indications

  • management of hypertension for end-organ protection

Pharmacology

  • dihydropyridine calcium-channel blocker
  • binds to dihydropyridine binding sites of L-type voltage dependent calcium channel
  • inhibits Ca2+ influx across the cell membranes of vascular smooth muscle cells
  • inhibits Ca2+ influx via N-type voltage dependent calcium channels in sympathetic nerve cell membrane
  • causes vasodilation
  • inhibits release of norepinephrine from sympathetic nerve terminals
  • suppresses reflex increase in heart rate after blood pressure reduction

Dosage & Administration

  • 5-10 mg once daily after breakfast for adults
  • maximum dose of 20 mg once daily for adults
  • safety in pediatric patients not established
  • dose initiation with lowest possible dose for elderly

Interaction

  • interacts with other anti-hypertensive medications
  • interacts with antipsychotics causing hypotension
  • interacts with quinidine, carbamazepine, phenytoin, rifampicin, cimetidine, and erythromycin

Contraindications

  • contraindicated in patients with known sensitivity to Cilnidipine or any excipients
  • contraindicated in patients with cardiogenic shock, recent MI, acute unstable angina, and severe aortic stenosis

  • peripheral edema

  • GI disturbances
  • increased micturition frequency
  • lethargy
  • eye pain

Pregnancy & Lactation

  • should not be administered to pregnant women or women with possibilities of being pregnant
  • advisable to avoid administration to nursing mothers
  • discontinuation advised if administration is indispensable

Precautions & Warnings

  • administered with care in patients with serious hepatic dysfunction
  • administered with care in patients with history of serious adverse reactions to calcium antagonists
  • dosage should be gradually decreased during discontinuation under close observation
