Indications

  • Management of hypertension
  • Treatment of congestive heart failure
  • Prevention of cardiovascular events
  • Reduction of risk of stroke and myocardial infarction
  • Treatment of diabetic nephropathy

Pharmacology

  • Ramipril is an ACE inhibitor
  • Blocks the conversion of angiotensin I to angiotensin II
  • Decreases plasma angiotensin II
  • Leads to decreased vasopressor activity
  • Effective in management of heart failure

Dosage

  • Initial dose for hypertension: 1.25-2.5 mg once daily
  • Maintenance dosage for adults: 2.5-20 mg daily
  • Initial dose for heart failure after myocardial infarction: 2.5 mg twice daily
  • Dosage in renal impairment: 1.25-5 mg daily

Administration

  • Tablets must be swallowed with liquid
  • Absorption not significantly affected by food
  • Can be taken before, during, or after a meal

Interaction

  • Concomitant administration with diuretics may lead to hypotension
  • Concomitant therapy with lithium may increase serum lithium concentration
  • NSAIDs may reduce antihypertensive effect

Contraindications

  • Hypersensitivity to ramipril or ACE inhibitors
  • History of angioedema
  • Concomitant use with sacubitril/valsartan
  • Pregnancy and breastfeeding

  • Angioneurotic oedema
  • Hyperkalaemia

Pregnancy & Lactation

  • Must not be taken during pregnancy
  • Pregnancy must be excluded before starting treatment
  • Not recommended during breastfeeding

Precautions & Warnings

  • Use with caution in patients with impaired renal function
  • Use with caution in patients with hyperkalaemia
  • Use with caution in patients with hypotension
  • Use with caution in patients with impaired hepatic function

Use in Special Populations

  • Elderly: reduced initial dose may be needed
  • Hepatic impairment: initiate treatment under close medical supervision
  • Renal impairment: adjust dose according to creatinine clearance
  • Patients pretreated with a diuretic: consider discontinuation or reduction of diuretic dose

Overdose Effects

  • Excessive peripheral vasodilatation
  • Marked hypotension and shock
  • Bradycardia
  • Electrolyte disturbances
  • Renal failure
