Indications
- Hypertension
- Congestive heart failure
- Treatment after acute myocardial infarction
- Glomerular or incipient nephropathy

Pharmacology
Ramipril is an angiotensin converting enzyme (ACE) inhibitor, which blocks the conversion of angiotensin I to angiotensin II, leading to decreased vasopressor activity and decreased aldosterone secretion. It is effective in managing heart failure and reducing the risk of stroke, myocardial infarction, and death from cardiovascular events.
Dosage
- Adjusted according to patient tolerance and response
- Initial dose for hypertension: 1.25-2.5 mg once daily
- Dosage in renal impairment: adjusted based on tolerance and BP response
- Dosage in heart failure and renal impairment: may be increased based on clinical response and tolerability
Administration
Tablets must be swallowed with sufficient liquid and not chewed or crushed. Absorption is not significantly affected by food. Can be taken before, during, or after a meal.
Interaction
- Concomitant administration with diuretics may lead to serious hypotension
- Concomitant therapy with lithium may increase serum lithium concentration
- Reduction in BP may affect ability to drive and operate machinery
- NSAIDs may reduce antihypertensive effect and cause renal function deterioration
Contraindications
- Hypersensitivity to ramipril or any other ACE inhibitor
- History of angioedema
- Concomitant use with sacubitril/valsartan therapy
- Haemodynamically relevant renal artery stenosis
- Hypotensive or haemodynamically unstable states
- With aliskiren-containing medicines in patients with diabetes or moderate to severe renal impairment
- With angiotensin II receptor antagonists (AIIRAs) in patients with diabetic nephropathy
- During pregnancy

Pregnancy & Lactation
Must not be taken during pregnancy. Should be discontinued if the patient intends to become pregnant. Not recommended during breastfeeding.
Precautions & Warnings
- Use with caution in patients with impaired renal function
- Use with caution in patients with hyperkalaemia
- Use with caution in patients with hypotension
- Use with caution in patients with impaired hepatic function
Use in Special Populations
- Elderly: consider reduced initial dose
- Hepatic impairment: initiate treatment under close medical supervision
- Renal impairment: consider reduced initial dose in certain cases
- Patients pretreated with a diuretic: consider discontinuation or reduction of diuretic dose
Overdose Effects
- Excessive peripheral vasodilatation
- Bradycardia
- Electrolyte disturbances
- Renal failure
