Indications

  • Prophylaxis and chronic treatment of asthma
  • Acute prevention of Exercise-Induced Bronchoconstriction (EIB)
  • Relief of symptoms of Allergic Rhinitis (AR): Seasonal & Perennial Allergic Rhinitis

Pharmacology

  • Selective and orally active leukotriene receptor antagonist
  • Inhibits the cysteinyl leukotriene receptor (CysLT1)
  • Correlated with the pathophysiology of asthma & allergic rhinitis
  • Affects airway edema, smooth muscle contraction, and cellular activity associated with the inflammatory process

Dosage & Administration

  • Adult dosage: Montelukast 10 mg tablet once daily
  • Pediatric dosage: 4 mg oral granules once daily for children aged 6 months to 5 years
  • No dosage adjustment required for hepatic or renal insufficiency
  • Similar pharmacokinetic profile in elderly and younger adults

Interaction

  • No clinically important effects on the pharmacokinetics of theophylline, prednisone, prednisolone, oral contraceptives, terfenadine, digoxin, and warfarin
  • Used concomitantly with a wide range of commonly prescribed drugs without evidence of clinical adverse interactions

Contraindications

  • Hypersensitivity to any component of the product

Side Effects

  • Common: Diarrhoea, fever, gastrointestinal discomfort, headache, nausea, vomiting, skin reactions, upper respiratory tract infection
  • Uncommon: Anxiety, depression, dizziness, drowsiness, malaise, muscle complaints, seizures, sleep disorders
  • Rare: Angioedema, hallucination, hepatic disorders, memory loss, suicidal tendencies, tremor

Pregnancy & Lactation

  • Crosses the placenta in rats and rabbits
  • Use during pregnancy only if clearly needed
  • Caution in nursing mothers

Precautions & Warnings

  • Not for use in the reversal of bronchospasm in acute asthma attacks
  • Appropriate rescue medication should be available
  • Therapy can be continued during acute exacerbations of asthma
  • Not to be abruptly substituted for inhaled or oral corticosteroids
  • Not to be used as monotherapy for the treatment of exercise-induced bronchospasm
  • Continued avoidance of aspirin or non-steroidal anti-inflammatory agents in patients with known aspirin sensitivity

Overdose Effects

  • No adverse experiences in the majority of overdosage reports
  • Adverse experiences consistent with the safety profile of the product
  • Usual supportive measures should be employed in the event of overdose
