Indications
- Prophylaxis and chronic treatment of asthma
- Acute prevention of Exercise-Induced Bronchoconstriction (EIB)
- Relief of symptoms of Allergic Rhinitis (AR): Seasonal & Perennial Allergic Rhinitis
Pharmacology
- Selective and orally active leukotriene receptor antagonist
- Inhibits the cysteinyl leukotriene receptor (CysLT1)
- Correlated with the pathophysiology of asthma & allergic rhinitis
- Affects airway edema, smooth muscle contraction, and cellular activity associated with the inflammatory process
Dosage & Administration
- Adult dosage: Montelukast 10 mg tablet once daily
- Pediatric dosage: 4 mg oral granules once daily for children aged 6 months to 5 years
- No dosage adjustment required for hepatic or renal insufficiency
- Similar pharmacokinetic profile in elderly and younger adults
Interaction
- No clinically important effects on the pharmacokinetics of theophylline, prednisone, prednisolone, oral contraceptives, terfenadine, digoxin, and warfarin
- Used concomitantly with a wide range of commonly prescribed drugs without evidence of clinical adverse interactions
Contraindications
- Hypersensitivity to any component of the product
Side Effects
- Common: Diarrhoea, fever, gastrointestinal discomfort, headache, nausea, vomiting, skin reactions, upper respiratory tract infection
- Uncommon: Anxiety, depression, dizziness, drowsiness, malaise, muscle complaints, seizures, sleep disorders
- Rare: Angioedema, hallucination, hepatic disorders, memory loss, suicidal tendencies, tremor
Pregnancy & Lactation
- Crosses the placenta in rats and rabbits
- Use during pregnancy only if clearly needed
- Caution in nursing mothers
Precautions & Warnings
- Not for use in the reversal of bronchospasm in acute asthma attacks
- Appropriate rescue medication should be available
- Therapy can be continued during acute exacerbations of asthma
- Not to be abruptly substituted for inhaled or oral corticosteroids
- Not to be used as monotherapy for the treatment of exercise-induced bronchospasm
- Continued avoidance of aspirin or non-steroidal anti-inflammatory agents in patients with known aspirin sensitivity
Overdose Effects
- No adverse experiences in the majority of overdosage reports
- Adverse experiences consistent with the safety profile of the product
- Usual supportive measures should be employed in the event of overdose
