Indications
- Treatment of hypertension
- Management of congestive heart failure
- Reduction of risk of myocardial infarction and stroke
- Treatment of renal complications in diabetic and non-diabetic patients
Pharmacology
- Angiotensin converting enzyme (ACE) inhibitor
- Blocks conversion of angiotensin I to angiotensin II
- Decreased vasopressor activity and aldosterone secretion
- Long-acting and well-tolerated
Dosage
- Initial dose range for hypertension: 1.25-2.5 mg once daily
- Maintenance dosage range for hypertension: 2.5-20 mg daily
- Initial dose for congestive heart failure after myocardial infarction: 2.5 mg twice daily
Administration
- Tablets must be swallowed whole with sufficient liquid
- Absorption not significantly affected by food
Interaction
- Contraindicated with diuretics due to risk of hypotension
- Concomitant therapy with lithium may increase serum lithium concentration
- NSAIDs may reduce antihypertensive effect
Contraindications
- Hypersensitivity to ramipril or any other ACE inhibitor
- History of angioedema
- Concomitant use with sacubitril/valsartan or aliskiren-containing medicines
- In pregnancy and with haemodynamically relevant renal artery stenosis
Side Effects
- Commonly reported side effects: dizziness, headache, fatigue
- Less frequent side effects: symptomatic hypotension, cough, nausea
Pregnancy & Lactation
- Contraindicated during pregnancy
- Not recommended during breastfeeding
Precautions & Warnings
- Use with caution in impaired renal function, hyperkalaemia, hypotension, and impaired hepatic function
- Reduced initial dose for elderly patients
Use in Special Populations
- Consider reduced initial dose for elderly and patients with renal impairment
- Initiate treatment in hepatic impairment under close medical supervision
Overdose Effects
- May cause excessive peripheral vasodilatation with marked hypotension
- Management includes detoxification and administration of adrenergic agonists

- Store at 30°C or below, protect from light
- Do not use after the expiry date
