Dosage and administration
- MDD:
- Starting Dose: 40 mg/day to 60 mg/day
- Target Dose: Acute: 40 mg/day to 60 mg/day; Maintenance: 60 mg/day
- Maximum Dose: 120 mg/day
- GAD:
- Starting Dose: 60 mg/day
- Target Dose: 60 mg/day
- Maximum Dose: 120 mg/day
- DPNP:
- Starting Dose: 60 mg/day
- Target Dose: 60 mg/day
- Maximum Dose: 60 mg/day
- Fibromyalgia:
- Starting Dose: 30 mg/day
- Target Dose: 60 mg/day
- Maximum Dose: 60 mg/day
- Chronic Musculoskeletal Pain:

- Other:
- Some patients may benefit from starting at 30 mg once daily
- No evidence for doses greater than 60 mg/day conferring additional benefit
- Gradual dose reduction recommended to avoid discontinuation symptoms
Interaction
- Metabolism involving CYP1A2 and CYP2D6 isozymes
- Effects of co-administration with inhibitors of CYP1A2 and CYP2D6
Contraindications
- Hypersensitivity to the drug or inactive ingredients
- Unapproved for treating bipolar depression
- Not recommended for patients with substantial alcohol use or chronic liver disease
- Caution in patients with controlled narrow-angle glaucoma

- Hyperhidrosis

- Slight increase in blood pressure
- No clinically significant differences in heart intervals observed
Pregnancy and lactation

- Labor and delivery: Effects unknown; use only if benefits outweigh risks
- Lactation: Unknown if excreted into human milk; not recommended during nursing
Precautions and warnings
- Close monitoring for clinical worsening and unusual changes in behavior during initial months of therapy
- Blood pressure measurement prior to treatment and periodically throughout
- Caution for bleeding risk when used with certain drugs
- Use cautiously in patients with a history of mania or seizure disorder

Overdose effects
- Limited clinical experience with overdose
- No specific antidote
- General measures for managing overdose recommended
- Caution in inducing emesis
- Activated charcoal may be useful in limiting absorption
