Indications
- Chronic management of hyperuricemia in patients with gout
- Not recommended for treatment of asymptomatic hyperuricemia
Pharmacology
- Febuxostat is a non-purine, selective xanthine oxidase (XO) inhibitor
- Decreases serum uric acid level by inhibiting xanthine oxidase
- Not expected to inhibit other enzymes involved in purine and pyrimidine synthesis and metabolism at therapeutic concentrations
Dosage & Administration
- Recommended starting dose: Febuxostat 40 mg tablet once daily
- Dose adjustment for patients based on serum uric acid levels
- Recommended doses for tumor lysis syndrome and gout flares
- No dose adjustment needed for renal or hepatic impairment
Interaction
- Inhibition of XO by Xanuric may cause increased plasma concentrations of certain drugs
- Co-administration with specific drugs and enzymes may require monitoring
- Potential impact on Uridine Glucuronosyl Transferase (UGT) enzymes
Contraindications
- Contraindicated in patients being treated with azathioprine, mercaptopurine, or theophylline
Side Effects
- Commonly reported adverse reactions: gout flares, liver function abnormalities, diarrhea, nausea, headache, rash, oedema
- Rare serious hypersensitivity reactions observed
- Most adverse reactions are mild or moderate in severity
Pregnancy & Lactation
- Pregnancy Category C
- Caution advised when used during pregnancy and lactation
Precautions & Warnings
- Increase in gout flares observed during initiation of treatment
- Prophylactic therapy may be beneficial for up to six months
- Higher rate of cardiovascular thromboembolic events observed in clinical trials
- Monitor for signs and symptoms of MI and stroke
- Transaminase elevations observed in Xanuric-treated patients
Overdose Effects
- Studied in healthy subjects in doses up to 300 mg daily for seven days without evidence of dose-limiting toxicities
