Indications

Pharmacology

  • ACE inhibitor
  • Vasoconstrictor substance blocker
  • Decreased vasopressor activity
  • Long-acting

Dosage

  • Adjustable according to tolerance and response
  • Initial dose for hypertension: 1.25-2.5 mg once daily
  • Initial dose for heart failure post-myocardial infarction: 2.5 mg twice daily
  • Initial dose for prevention of cardiovascular events: 2.5 mg once daily

Administration

  • Swallow with liquid
  • Do not chew or crush
  • Can be taken before, during, or after a meal

Interaction

  • Contraindicated with diuretics
  • May interact with lithium and NSAIDs
  • Alcohol and hypotension interaction

Contraindications

  • Hypersensitivity to ramipril or other ACE inhibitors
  • History of angioedema
  • Concomitant use with certain therapies or conditions
  • Pregnancy
  • Extracorporeal treatments
  • Renal artery stenosis

Precautions & Warnings

  • Caution in patients with impaired renal function, hyperkalemia, hypotension, and impaired hepatic function
  • Reduced initial dose for the elderly and patients with hepatic impairment

Use in Special Populations

  • Elderly: Reduced initial dose
  • Hepatic impairment: Initiate treatment under close supervision
  • Renal impairment: Adjusted initial dose based on creatinine clearance
  • Patients pretreated with a diuretic: Consider discontinuation or dose reduction

Overdose Effects

  • Excessive peripheral vasodilatation
  • Bradycardia
  • Electrolyte disturbances
  • Renal failure
