Description
- Methylprednisolone, a glucocorticoid with anti-inflammatory effects
- Used for various conditions including endocrine, rheumatic, collagen, dermatological, allergic, ophthalmic, gastrointestinal, respiratory, hematologic, neoplastic, edematous, and miscellaneous disorders
Dosage and Administration
- Administered by IM or IV injection
- Recommended initial dosage varies from 10 to 40 mg depending on the clinical problem
- High-dose therapy may require 30 mg/kg administered intravenously over at least 30 minutes
- Pediatric initial dose varies depending on the specific disease being treated
Indications
- Primary or secondary adrenocortical insufficiency
- Acute adrenocortical insufficiency
- Rheumatoid arthritis
- Acute and subacute bursitis
- PSoriatic arthritis
- Ankylosing spondylitis
- Pemphigus
- Severe psoriasis
- Allergic states including asthma and dermatitis
- Severe acute and chronic allergic and inflammatory processes involving the eye
- Ulcerative colitis
- Tuberculosis
- Acquired hemolytic anemia
- Leukemias and lymphoma
- Nephrotic syndrome
- Tuberculous meningitis
Interaction
- Aminoglutethimide may lead to a loss of corticosteroid-induced adrenal suppression
- Concomitant use of corticosteroids and warfarin usually results in inhibition of response to warfarin
- Corticosteroids may increase blood glucose concentrations, requiring dosage adjustments of antidiabetic agents
- Corticosteroids may suppress reactions to skin tests
- Patients on prolonged corticosteroid therapy may exhibit a diminished response to vaccines
Contraindications
- Systemic fungal infections
- Known hypersensitivity to the product and its constituents
- Intramuscular corticosteroid preparations are contraindicated for idiopathic thrombocytopenic purpura
Side Effects
- Fluid and electrolyte disturbances
- Muscle weakness
- Peptic ulcer with possible perforation and hemorrhage
- Impaired wound healing
- Increased intracranial pressure
- Development of Cushingoid state
- Posterior subcapsular cataracts
- Negative nitrogen balance due to protein catabolism
Pregnancy and Lactation
- Corticosteroids have been shown to be teratogenic in many species when given in doses equivalent to the human dose
- Systemically administered corticosteroids appear in human milk
Precautions and Warnings
- Sensitive to heat
- Lowest possible dose should be used to control the condition
- Complications of treatment with glucocorticoids are dependent on the size of the dose and the duration of treatment
- Pediatric patients may experience a decrease in their growth velocity
Use in Special Populations
- Pediatric patients should be carefully observed with frequent measurements
- Elderly patients may respond differently and should start at the low end of the dosing range
