Indications
- Hypertension
- Treatment of essential hypertension
- Chronic heart failure (CHF)
- Treatment of stable mild and moderate chronic heart failure in elderly patients.
Pharmacology
- β adrenergic receptor blocking agent
- Decreased heart rate
- Decreased myocardial contractility
- Diminution of tonic sympathetic outflow to the periphery from cerebral vasomotor centers
- Suppression of renin activity
- Vasodilation and decreased peripheral vascular resistance
Pharmacokinetics
- Metabolized by CYP2D6
- Effective half-life of about 12 hours in extensive metabolizers
- Effective half-life of about 19 hours in poor metabolizers
- Plasma protein binding of nebivolol is approximately 98%
- Independent of nebivolol concentrations
Dosage & Administration
- Recommended starting dose is 5 mg once daily
- Can be increased at 2-week intervals up to 40 mg
- More frequent dosing regimen unlikely to be beneficial
Interactions
- Concurrent use with myocardial depressants or inhibitors of AV conduction
- Avoid concomitant use with other β-blockers
- Cautious use with catecholamine-depleting drugs
- Use caution with CYP2D6 inhibitors
Contraindications
- Severe bradycardia
- Heart block greater than first degree
- Cardiogenic shock
- Decompensated cardiac failure
- Sick sinus syndrome
- Severe hepatic impairment
- Hypersensitivity to any component of this product
Side Effects
- Headache

- Bradycardia
Precautions & Warnings
- Patients with coronary artery disease should avoid abrupt discontinuation of therapy
- Administer cautiously in patients with compensated congestive heart failure
- Not studied in patients with recent myocardial infarction
- Patients with bronchospastic diseases should not receive β-blockers
- Cautious perioperative use with anesthetic agents
- May mask manifestations of hypoglycemia
- May mask clinical signs of hyperthyroidism
- Caution in patients with peripheral vascular disease
Use in Special Populations
- Use with caution in patients with severe renal impairment
- Use with caution in patients with moderate hepatic impairment
- Nebipres is contraindicated in patients with severe hepatic impairment
- Patients with a history of severe anaphylactic reactions may be more reactive
