Indications
- Bronchial asthma
- Bronchospasm
- Chronic obstructive pulmonary disease (COPD)
- Pulmonary disease with spastic bronchial component
Pharmacology
- Novel bronchodilator
- Structurally differs from Theophylline
- Selective inhibition of phosphodiesterase 4
- Relaxes bronchial smooth muscle
- Decreased affinities towards adenosine A1 and A2 receptors
- Inhibition of platelet activating factor (PAF)
- Inhibition of leukotriene production
Dosage & Administration
- 200 mg tablet for elderly two or three times daily
- 400 mg tablet for adults two or three times daily
- 400 mg tablet for children above 12 years two or three times daily
- 6-9 mg/kg body weight for children 6-12 years old
- 400 mg Doxophylline SR tablet once daily if required
- Physician's prescription is required
Interaction
- Not to be administered with other xanthine preparations
- Limit consumption of caffeine-containing beverages and food
- Caution with ephedrine or other sympathomimetic drugs
- Simultaneous administration may reduce hepatic clearance of xanthine derivatives
- Certain drugs may increase the clearance of xanthine derivatives
- Possible dosage adjustment for certain drug interactions
Contraindications
- Known hypersensitivity to Doxophylline or other xanthine derivatives
- Acute myocardial infarction
- Hypotension
- During lactation
Side Effects
- Nausea
- Vomiting
- Epigastric pain
- Headache
- Irritability
- Insomnia
- Tachycardia
- Extrasystoles
- Tachypnea
- Rare cases of hyperglycemia or albuminuria
- Severe cardiac arrhythmias in case of overdose
- Tonic-clonic seizure in case of overdose
Pregnancy & Lactation
- Animal tests show no interference with pre and postnatal growth
- Insufficient clinical evidence on use during pregnancy
- Contraindicated during lactation
- Evaluation of risk-benefit ratio for use during pregnancy
Precautions & Warnings
- Factors reducing hepatic clearance may increase plasma levels
- Monitoring of blood levels may be necessary in certain cases
- Caution in patients with cardiac disease, hypertension, or renal impairment
- Special caution in patients with congestive heart failure
- No risk of addiction or dependence
Overdose Effects
- Severe cardiac arrhythmias
- Tonic-clonic seizure
- Discontinuation and possible resumption at lower doses under physician's discretion
- Symptomatic treatment of cardiovascular collapse
