Indications
- Treatment of psoriatic arthritis
- Treatment of plaque psoriasis
- For patients candidates for phototherapy or systemic therapy
Pharmacology
- Inhibitor of type-4 cyclic nucleotide phosphodiesterase
- Increases intracellular levels of cAMP
- Inhibits production of proinflammatory mediators
- Restores homeostatic balance between pro- and anti-inflammatory signaling
Dosage & Administration
- Initial dosage titration for 5 days
- Recommended maintenance dosage of 30 mg twice daily
- Reduction of gastrointestinal symptoms with titration
- Can be administered without regard to meals
Interaction
- Strong cytochrome P450 enzyme inducers result in reduction of systemic exposure
- Avoid co-administration with Rifampin, Phenobarbital, Carbamazepine, Phenytoin
Contraindications
- Known hypersensitivity to Apremilast or excipients
Side Effects
- Frequent: nausea, diarrhea, headache
- Less frequent: upper respiratory tract infection, vomiting, nasopharyngitis, abdominal pain, hypersensitivity, gastroesophageal reflux disease, dyspepsia, fatigue, decreased appetite, cough, rash, insomnia
Pregnancy & Lactation
- Pregnancy Category C
- Presence of drug in lactating mouse milk
- Caution in nursing women
Precautions & Warnings
- Increased risk of depression
- Need to monitor for mood changes
- Weight decrease in psoriatic arthritis patients
- Evaluation of risks and benefits in case of adverse events
Use in Special Populations
- Safety and effectiveness not established in pediatric patients
