Indications

  • Hypertension
  • Congestive heart failure
  • Acute myocardial infarction treatment
  • Glomerular or incipient nephropathy treatment
  • Reduction in cardiovascular risk

Pharmacology

Angiotensin converting enzyme (ACE) inhibitor

Dosage

  • Initial dose 1.25-2.5 mg for hypertension
  • Initial dose 2.5 mg twice daily for congestive heart failure after myocardial infarction
  • Recommended dose 2.5 mg once daily for prevention of major cardiovascular events

Administration

Tablets to be swallowed with liquid, not to be chewed or crushed

Interaction

  • Contraindicated with diuretics
  • May increase serum lithium concentration when used with lithium
  • May be affected by alcohol and NSAIDs

Contraindications

  • Hypersensitivity to ramipril or any other ACE inhibitor
  • History of angioedema
  • Concomitant sacubitril/valsartan therapy
  • Renal artery stenosis
  • Hypotensive or haemodynamically unstable states
  • Concomitant aliskiren-containing medicines in patients with diabetes or moderate to severe renal impairment
  • Concomitant angiotensin II receptor antagonists (AIIRAs) in diabetic nephropathy
  • During pregnancy

Use in Special Populations

  • Elderly: consider reduced initial dose
  • Hepatic impairment: initiate treatment under close medical supervision
  • Renal impairment: consider reduced initial dose based on creatinine clearance

Overdose Effects

  • Excessive peripheral vasodilatation
  • Hypotension
  • Bradycardia
  • Electrolyte disturbances
  • Renal failure
