Indications
- Hypertension
- Congestive heart failure
- Acute myocardial infarction treatment
- Glomerular or incipient nephropathy treatment
- Reduction in cardiovascular risk
Pharmacology
Angiotensin converting enzyme (ACE) inhibitor
Dosage
- Initial dose 1.25-2.5 mg for hypertension
- Initial dose 2.5 mg twice daily for congestive heart failure after myocardial infarction
- Recommended dose 2.5 mg once daily for prevention of major cardiovascular events
Administration
Tablets to be swallowed with liquid, not to be chewed or crushed
Interaction
- Contraindicated with diuretics
- May increase serum lithium concentration when used with lithium
- May be affected by alcohol and NSAIDs
Contraindications
- Hypersensitivity to ramipril or any other ACE inhibitor
- History of angioedema
- Concomitant sacubitril/valsartan therapy
- Renal artery stenosis
- Hypotensive or haemodynamically unstable states
- Concomitant aliskiren-containing medicines in patients with diabetes or moderate to severe renal impairment
- Concomitant angiotensin II receptor antagonists (AIIRAs) in diabetic nephropathy
- During pregnancy

Use in Special Populations
- Elderly: consider reduced initial dose
- Hepatic impairment: initiate treatment under close medical supervision
- Renal impairment: consider reduced initial dose based on creatinine clearance
Overdose Effects
- Excessive peripheral vasodilatation
- Hypotension
- Bradycardia
- Electrolyte disturbances
- Renal failure
