Indications
- Hypertension
- Congestive heart failure
- Treatment after acute myocardial infarction
- Glomerular or incipient nephropathy
- Risk reduction for myocardial infarction, stroke, or cardiovascular death
Pharmacology
- Angiotensin converting enzyme (ACE) inhibitor
- Decreased vasopressor activity
- Decreased aldosterone secretion
- Management of heart failure
- Risk reduction for stroke, myocardial infarction, and death from cardiovascular events
- Long-acting and well-tolerated
Dosage
- Initial dose of 1.25-2.5 mg once daily for hypertension in adults
- Maintenance dosage of 2.5-20 mg daily for hypertension
- Initial dose of 2.5 mg twice daily for congestive heart failure after myocardial infarction
- Prevention of major cardiovascular events dosage: 2.5 mg once daily for the first week, 5 mg once daily for the following 3 weeks, increased to 10 mg once daily as tolerated
- Adjusted dosage for renal impairment
Administration
To be swallowed whole with liquid; absorption not affected by food
Interaction
- Concomitant administration with diuretics may cause hypotension
- Concomitant therapy with lithium may increase serum lithium concentration
- BP reduction may affect ability to drive and operate machinery
- NSAIDs may reduce the antihypertensive effect and cause deterioration of renal function
Contraindications
- Hypersensitivity to ramipril or any other ACE inhibitor
- History of angioedema
- Concomitant use with sacubitril/valsartan therapy
- Haemodynamically relevant renal artery stenosis
- Hypotensive or haemodynamically unstable states
- Concomitant use with aliskiren-containing medicines in patients with diabetes or with moderate to severe renal impairment
- Concomitant use with angiotensin II receptor antagonists in patients with diabetic nephropathy
- During pregnancy
- Concomitant use with extracorporeal treatments leading to contact of blood with negatively charged surfaces

Precautions & Warnings
- Use with caution in patients with impaired renal function
- Use with caution in patients with hyperkalaemia
- Use with caution in patients with hypotension
- Use with caution in patients with impaired hepatic function
Use in Special Populations
- Elderly: Consider reduced initial dose
- Hepatic impairment: Initiate treatment under close medical supervision
- Renal impairment: Consider reduced initial dose
- Patients pretreated with a diuretic: Consider discontinuation of diuretic before starting treatment
Overdose Effects
- Excessive peripheral vasodilatation
- Marked hypotension
- Shock
- Bradycardia
- Electrolyte disturbances
- Renal failure
