Indications
- Hypertension
- Congestive heart failure
- Treatment after acute myocardial infarction
- Glomerular or incipient nephropathy
- Risk reduction for myocardial infarction, stroke, or cardiovascular death
Pharmacology
- ACE inhibitor
- Blocks conversion of angiotensin I to angiotensin II
- Decreases vasopressor activity
- Decreases aldosterone secretion
- Long-acting
- Well tolerated
Dosage
- Adjusted according to patient tolerance and response
- Initial dose for hypertension: 1.25-2.5 mg once daily
- Maintenance dosage: 2.5-20 mg daily
- Initial dose for congestive heart failure after myocardial infarction: 2.5 mg twice daily
- Prevention of major cardiovascular events: 2.5 mg once daily for first week, then 5 mg once daily
- Dosage in renal impairment varies
Administration
- Tablets to be swallowed with sufficient liquid
- Not to be chewed or crushed
- Absorption not significantly affected by food
Interaction
- Concomitant administration with diuretics may lead to serious hypotension
- Concomitant therapy with lithium may increase serum lithium concentration
- Reduction in BP may affect ability to drive and operate machinery
- NSAIDs may reduce antihypertensive effect and cause renal function deterioration
Contraindications
- Hypersensitivity to ramipril or any ACE inhibitor
- History of angioedema
- Concomitant use with sacubitril/valsartan therapy
- Haemodynamically relevant renal artery stenosis
- Hypotensive or haemodynamically unstable states
- Concomitant use with aliskiren-containing medicines or AIIRAs in certain patients
- During pregnancy
- During extracorporeal treatments with negatively charged surfaces

Precautions & Warnings
- Use with caution in patients with impaired renal function, hyperkalaemia, hypotension, and impaired hepatic function
Use in Special Populations
- Reduced initial dose for elderly
- Initiate treatment in hepatic impairment only under close medical supervision
- Adjust initial dose for patients with renal impairment or pretreated with a diuretic
Overdose Effects
- Excessive peripheral vasodilatation
- Marked hypotension
- Shock
- Bradycardia
- Electrolyte disturbances
- Renal failure
