Indications
- Control of acute asthma
- Management of chronic asthma (symptomatic and prophylactic treatment)
- Controlling nocturnal asthma and early morning wheezing
- Management of chronic obstructive lung disease (chronic bronchitis and emphysema)
- Acute exacerbation of chronic obstructive lung disease
- Control of apnea of pre-maturity
Pharmacology
Bronchodilator with smooth muscle relaxation and suppression of airway response to stimuli. Increases diaphragmatic muscle contraction. Half-life ranges from 3 to 9 hours in adult nonsmokers with uncomplicated asthma.
Dosage & Administration
- Dose individualized based on peak serum theophylline concentration measurements
- Sustained release preparations may be administered every 12 hours in adults, every 8 hours in some children
- Recommended maintenance dose within accepted therapeutic range for different age groups
Interactions
Should not be used concurrently with other preparations containing xanthine derivatives. Clearance of Theo-fast is affected by various drugs, necessitating dosage adjustments.
Contraindications
Hypersensitivity to Theophylline or any other component of the product.
Side Effects
- Rare at normal dosage
- May include gastrointestinal discomfort, headache, nausea, insomnia, hypotension
- CNS stimulation and diuresis may occur, especially in children.
Pregnancy & Lactation
Not known whether Theophylline can cause fetal harm; should be given to pregnant women only if clearly needed. Excreted into breast milk and may cause irritability in nursing infants.
Precautions & Warnings
Careful consideration needed for interacting drugs and physiologic conditions that can alter Theo-fast clearance. Dosage adjustment required prior to initiation and increases in dose.
Use in Special Populations
Clearance of Theo-fast is very low in neonates. Careful attention to dosage selection and monitoring of serum concentration required in pediatric patients.
