Indications
- Treatment of advanced or metastatic breast cancer
- Indicated in combination with capecitabine or letrozole
- Limitations of use: Patients should have disease progression on trastuzumab prior to initiation
Pharmacology
- Kinase inhibitor of EGFR and HER2 receptors
- Inhibits ErbB-driven tumor cell growth
- Additive effect with 5-FU
- Non-cross-resistance with trastuzumab
- Metabolism by CYP3A4, CYP3A5, CYP2C19, and CYP2C8
- Highly bound to albumin and alpha-1 acid glycoprotein
Dosage & Administration
- Recommended dose of 1,250 mg orally once daily
- In combination with capecitabine or letrozole
- Dosing regimen in repeating 21-day cycle
- Capecitabine administered orally in 2 doses
- Should be taken at least one hour before or after a meal
Interaction
- Inhibition of CYP3A4, CYP2C8, and P-glycoprotein
- Effects on drug-metabolizing enzymes and transport systems
- Interaction with midazolam, paclitaxel, and digoxin
- Effects of concomitant use of strong inhibitors or inducers of CYP3A4 enzymes
Contraindications
- Contraindicated in patients with known severe hypersensitivity
Side Effects
- Common adverse reactions include diarrhea, palmar-plantar erythrodysesthesia, nausea, rash, vomiting, and fatigue
Pregnancy & Lactation
- Potential fetal harm if administered to pregnant women
- Lactating women advised not to breastfeed during treatment and for 1 week after last dose
Precautions & Warnings
- Decreased left ventricular ejection fraction
- Hepatotoxicity
- Diarrhea
- Interstitial lung disease/pneumonitis
- QT prolongation
- Severe cutaneous reactions
Use in Special Populations
- Dose modification guidelines for cardiac events, hepatic impairment, diarrhea, and concomitant CYP3A4 inhibitors/inducers
Overdose Effects
- No known antidote for overdoses
- Maximum oral doses administered in clinical trials
- General supportive measures recommended for treatment of overdose
