Indications
- Lowering blood pressure
- Managing congestive heart failure
- Treating acute myocardial infarction
- Treating glomerular or incipient nephropathy

Pharmacology
- ACE inhibitor
- Blocks angiotensin I conversion to angiotensin II
- Reduces vasopressor activity
- Reduces aldosterone secretion
- Long-acting and well-tolerated
Dosage
- Initial dose of 1.25-2.5 mg once daily
- Maintenance dosage of 2.5-20 mg daily
- Dose adjustment for renal impairment
- Tablets to be swallowed with liquid
- Absorption not affected by food
Administration
- Swallow tablets with sufficient liquid
- Do not chew or crush
- May be taken before, during, or after a meal
Interaction
- Concomitant use with diuretics
- Concomitant therapy with lithium
- Effect on ability to drive and operate machinery
- Interaction with NSAIDs
Contraindications
- Hypersensitivity to ramipril or ACE inhibitor
- History of angioedema
- Concomitant use with sacubitril/valsartan therapy
- Renal artery stenosis
- Hypotensive or haemodynamically unstable states
- Concomitant use with aliskiren-containing medicines or AIIRAs
- During pregnancy

- Use with caution in impaired hepatic function
Use in Special Populations
- Reduced initial dose in elderly
- Initiate treatment in hepatic impairment under close supervision
- Initial dose based on creatinine clearance in renal impairment
- Consideration for patients pretreated with a diuretic
Overdose Effects
- Excessive peripheral vasodilatation
- Marked hypotension
- Bradycardia
- Electrolyte disturbances
- Renal failure
- Management protocols for detoxification and support
