Indications
- Monotherapy for partial-onset seizures in adults and adolescents
- Adjunctive therapy for partial-onset seizures in adults, adolescents, children, and infants
- Treatment of myoclonic seizures in adults and adolescents
- Treatment of primary generalized tonic-clonic seizures in adults and adolescents
Mechanism of Action
- Unknown precise mechanism
- Minimal activity in submaximal stimulation and threshold tests
- Inhibition of burst firing without affecting normal neuronal excitability
- Selective prevention of epileptiform burst firing and seizure activity propagation
Dosage
- Recommended starting dose of 250 mg twice daily for adults and adolescents from 16 years of age
- Initial therapeutic dose of 500 mg twice daily after two weeks
- Maximum dose of 1500 mg twice daily for adults and adolescents
- Dose adjustments for pediatric population based on age and weight
Administration
- Oral administration
- To be swallowed with sufficient liquid
- To be taken with or without food
- Daily dose to be divided in two equal doses
Interactions
- No influence on serum concentrations of existing antiepileptic drugs
- Potential interaction with probenecid, methotrexate, and laxatives
- No alteration of drug absorption by food, but reduced rate of absorption
Contraindications
- Hypersensitivity to the active substance or other pyrrolidone derivatives or excipients
Side Effects
- Common adverse reactions in adults and pediatric patients
- Incidence of somnolence, asthenia, infection, dizziness, fatigue, aggression, nasal congestion, decreased appetite, and irritability
Pregnancy & Lactation
- Not recommended during pregnancy and breastfeeding
- Excreted in human breast milk
- Unknown impact on fertility in humans
Precautions & Warnings
- Dose adjustment for renal impairment
- Rare cases of decreased blood cell counts
- Association with suicide, suicidal ideation, and behaviour
- Not adapted for use in infants and children under 6 years
Use in Special Populations
- Dose adjustment in elderly and patients with compromised renal function
- No dose adjustment needed in patients with mild to moderate hepatic impairment
Overdose Effects
- Observations after overdoses include somnolence, agitation, respiratory depression, and coma
- No specific antidote
- Symptomatic treatment may include haemodialysis
