Pharmacology

  • Preparation of tranexamic acid (trans-4 aminomethyl-cyclohexanecarboxylic acid)
  • Strong antifibrinolytic action
  • Inhibition of plasminogen activation
  • Low acute and chronic toxicity
  • Well absorbed by oral route
  • Excreted mainly by renal route
  • No interference with clotting processes

Dosage & Administration

  • Usual dose for adults: 500-1000 mg 3 times daily
  • Recommended daily doses for prophylaxis: 0.5-1 gm orally, 500 mg by the parenteral route
  • Increased oral dose for therapy of hemorrhagic manifestations: 1-3 gm
  • Dosage for children based on body weight

Interaction

  • Incompatible with solutions containing penicillins
  • Antagonized by thrombolytic drugs like Streptokinase & Urokinase
  • Potential for increased thrombus formation with concomitant administration of estrogen-containing drugs

Contraindications

  • Known individual hypersensitivity
  • Thromboembolic disease
  • Arterial and venous thrombosis
  • Endocavitary hemorrhages
  • Serious kidney failure

Pregnancy & Lactation

  • Should not be administered during known and presumed pregnancy
  • Passes into breast milk to a low concentration unlikely to cause antifibrinolytic effect in the infant

  • Use in cases of hyperfibrinolysis
  • Prophylactic treatment timing
  • Prolonged therapy after hemorrhage disappearance
  • Dose adjustment in hematuria
  • Caution in renal insufficiency, cardiopathic, and hepatopathic subjects
