Indications
- Hypertension

Pharmacology
- ACE inhibitor
- Blocks conversion of angiotensin I to angiotensin II
- Decreases vasopressor activity
- Decreases aldosterone secretion
- Long-acting
Dosage
- Initial dose of 1.25-2.5 mg for hypertension
- Initial dose of 2.5 mg twice daily for heart failure post-myocardial infarction
- Dosage adjustment based on renal impairment
- Swallowed with liquid
- Can be taken with or without food
Administration
- Tablets must be swallowed whole
- Absorption not affected by food
Interaction
- Concomitant use with diuretics may lead to hypotension and hyperkalemia
- Concomitant therapy with lithium may increase serum lithium concentration
Contraindications
- Hypersensitivity to ramipril or ACE inhibitors
- History of angioedema
- Concomitant use with sacubitril/valsartan
- Renal artery stenosis
- Hypotensive or haemodynamically unstable states
- Concomitant use with aliskiren-containing medicines or AIIRAs in certain conditions
- During pregnancy
- Concomitant use with extracorporeal treatments

Pregnancy and lactation
- Must not be taken during pregnancy
- Pregnancy must be avoided
- Not recommended during breastfeeding
Precautions and warnings
- Caution in impaired renal function
- Caution in hyperkalemia
- Caution in hypotension
- Caution in impaired liver function
Special populations
- Elderly: consider reduced initial dose
- Hepatic impairment: initiate under close medical supervision
- Renal impairment: dose adjustment based on clearance rate
- Patients pretreated with diuretics: consider discontinuation or dose reduction
Overdose effects
- Excessive peripheral vasodilatation
- Bradycardia
- Electrolyte disturbances
- Renal failure
- Management: gastric lavage, adsorbents, sodium sulfate, adrenergic agonists, angiotensin II
