Indications

  • Hypertension
  • Congestive heart failure
  • Acute myocardial infarction
  • Glomerular nephropathy
  • Cardiovascular risk reduction

Pharmacology

Blocks the conversion of angiotensin I to angiotensin II, decreases plasma angiotensin II, reduces vasopressor activity, and decreases aldosterone secretion

Dosage

  • Adjustable according to patient tolerance and response
  • Usual initial dose 1.25-2.5 mg once daily for hypertension
  • 5 mg twice daily for congestive heart failure after myocardial infarction
  • 2.5 mg once daily for Prevention of major cardiovascular events

Administration

  • Tablets must be swallowed with liquid
  • Can be taken before, during, or after a meal

Interaction

  • May lead to hypotension and hyperkalemia with diuretics
  • May increase lithium serum concentration when used concomitantly with lithium
  • NSAIDs may reduce antihypertensive effect and cause renal function deterioration

Contraindications

  • Hypersensitivity to ramipril or other ACE inhibitors
  • History of angioedema
  • Concomitant use with sacubitril/valsartan therapy
  • Haemodynamically relevant renal artery stenosis
  • Hypotensive or haemodynamically unstable states
  • Concomitant use with aliskiren-containing medicines in certain patients

Pregnancy & Lactation

Precautions & Warnings

  • Use with caution in patients with impaired renal function, hyperkalaemia, hypotension, and impaired hepatic function

Use in Special Populations

  • Consider reduced initial dose for elderly patients
  • Initiate treatment with close medical supervision for patients with hepatic impairment

Overdose Effects

  • Excessive peripheral vasodilatation
  • Marked hypotension
  • Bradycardia
  • Electrolyte disturbances
  • Renal failure
