Indications

  • Irritable Bowel Syndrome with Constipation (IBS-C)
  • Chronic Idiopathic Constipation (CIC) in adults
  • Opioid-Induced Constipation (OIC) in adult patients with chronic, non-cancer pain

Pharmacology

  • Mode of Action: Lubiprostone is a locally-acting chloride-channel activator that increases intestinal secretion.
  • ClC-2 Chloride Channels: Specifically activates ClC-2 chloride channels to increase intestinal fluid secretion and motility.

Dosage & Administration

  • IBS with Constipation: 8 mcg twice daily orally with food and water
  • Hepatic Impairment: Dosage adjustment may be required for severely impaired hepatic function
  • Chronic Idiopathic Constipation & Opioid-induced Constipation: 24 mcg twice daily orally with food and water

Interaction

Possible dose-dependent decrease in efficacy in patients using diphenylheptane opioids

Contraindications

Contraindicated in patients with known or suspected mechanical gastrointestinal obstruction

Pregnancy & Lactation

  • Pregnancy Category: C
  • Excretion in Human Milk: Not known whether lubiprostone is excreted in human milk
  • Caution in Nursing Woman: Caution should be exercised when Lubiprostone is administered to a nursing woman

Precautions & Warnings

  • Nausea: Patients taking Lubigut may experience nausea
  • Diarrhea: Lubistone should not be prescribed to patients with severe diarrhea
  • Bowel Obstruction: Thorough evaluation to confirm absence of obstruction prior to therapy with Lubigut
