Medicine Details

Indications

  • Adjunct to diet and exercise in type 2 diabetes mellitus patients
  • Adjunct to glimepiride or metformin monotherapy
  • Replacement of combination therapy of glimepiride and metformin

Pharmacology

  • Sulfonylurea antidiabetic agent
  • Decreases blood glucose concentration
  • Stimulates insulin release from pancreatic beta cells
  • Improves sensitivity of beta cells to glucose stimulus
  • Reduces basal hepatic glucose production
  • Increases peripheral tissue sensitivity to insulin
  • Improves glucose uptake

Dosage

  • Governed by desired blood glucose level
  • Regular blood and urine glucose level measurements required
  • Starting dose not to exceed existing glimepiride or metformin doses
  • Maximum recommended dose: 8 mg glimepiride, 2000 mg metformin
  • Dose reduction or cessation to avoid hypoglycemia

Administration

  • Tablet must be swallowed whole
  • Must not be crushed or chewed

Interaction

  • Metabolized by cytochrome P450 2C9 (CYP2C9)
  • Potential interactions with various drugs including insulin, ACE inhibitors, corticosteroids, beta-blockers
  • Alcohol intake may potentiate or weaken blood-glucose-lowering action

Contraindications

  • Hypersensitivity to glimepiride, metformin, sulfonylureas, sulfonamides, or any excipients
  • Pregnant or breastfeeding women
  • Severe impairment of liver function or renal failure

  • Metformin excreted into milk in lactating rats

Precautions & Warnings

  • Increased risk of hypoglycemia in initial weeks of treatment
  • Careful monitoring in patients with G6PD-deficiency
  • Regular monitoring of TSH levels in hypothyroid patients

Use in Special Populations

  • Insufficient data for pediatric use
  • Renal function assessment before initiation of treatment
  • Dose adjustment in relation to declining renal function

Overdose Effects

  • Severe life-threatening hypoglycemia
  • Lactic acidosis
  • Pancreatitis
